To enroll in VCS Ohio, students must submit all of the following documents, as described in the admissions checklist, and attend a student orientation. If you need help, send a student enrollment inquiry or call (866) 501-9473.

Special education services are available at VCS Ohio.

Admissions Checklist:

APPLICATION FORM - Please be sure to complete the entire application (consists of four parts). Students may complete the online enrollment form or complete a paper application.

CONSENT FORM FOR STUDENT RECORDS RELEASE - A parent/guardian must sign a consent form that allows VCS to request the student’s records from the previous school attended.

PROOF OF RESIDENCY - This must be a utility bill, a copy of a current lease or a mortgage statement only.

BIRTH CERTIFICATE - A copy of the student’s birth certificate. A copy of a baptismal certificate, passport or naturalization papers are acceptable in lieu of a birth certificate.

TRANSCRIPT - Please provide transcripts for high school students and grade cards for students in kindergarten through eighth grade.

IMMUNIZATION HISTORY - Under the Ohio Revised Code, the parent or legal guardian is required to submit written evidence that his/her child has had all the required immunizations. It is the parent’s or guardian’s responsibility to furnish this information. According to Tony Patton, Manager of the Immunization Program for the Ohio Department of Health, the Ohio Revised Code 3313.671 says that all kindergartners from this year forward “must be vaccinated against varicella (chickenpox) disease,” and that “This will be a progressively implemented law; for the 2007-2008 school year, the varicella vaccine will be required for K - 1, and so forth.”

COPY OF CUSTODY ORDER OR DIVORCE DECREE (if applicable) - Under the Ohio Revised Code, proof of custody MUST be presented for any student for which custody has been determined by a court. Please provide adoption papers or other guardianship documents. If you’ve had a recent name change, please provide the appropriate documentation for that as well.

SPECIAL NEEDS DOCUMENTATION (if applicable) - Please provide the most recent Individual Election Plan (IEP) and Evaluation Team Report (ETR).
